Block Party Permit Application
A "Block Party" is a gathering organized by the residents of the neighborhood abutting the street to be closed, for their
enjoyment. A Block Party Permit must be obtained to close a public street to conduct a neighborhood block party.
Public Records Request
This form is used to process public records requests in accordance with the Oregon Public Records Law
(ORS Chapter 192).
Ride-Along Application
This form is to request permission to ride as a citizen observer with an officer of the Lake Oswego
Police Department.
Special Events Permit Application Special Events are gatherings/events that involve a large group of people (compared
to the usual occupancy of the site), and:
- Are advertised to the public and do not occur regularly on the site (LOC 20.06.500);
- Impact or use City streets (including curb or parking lane), sidewalks, and public rights-of-way, or
- Use City facilities or property (i.e. city buildings, parks, and parking lots.)
Examples include: road race, bike race, walk, parade, festival, market, carnival, and concert. (Special Event Permits must
be obtained.)
Movie / Photo Permit Production companies and photographers are required to obtain a Movie / Photo Permit whenever the filming / photo shoot
involves the closure or restriction of a street, right-of-way, or use of a City property or facility, e.g., park, building.

Because of the ever-increasing demand, we have found it necessary to charge $5.00 for each patch.
This form can be printed out, completed, and mailed in with payment.
Alarm Use Permit Application
Print this form, fill it out and return it to our Alarm Coordinator for consideration of an alarm permit.

Business Check Report Packet
If you are a merchant and have received a forged check returned to you from the bank, the Lake
Oswego Police Department requires that you present the original check with the Affidavit of Forgery
completed by the account holder, as well as the employee who completed the transaction with the suspect.
When this information is available, download the Business Check Report Packet from this site. Once
completed, business owners or representative may contact the Lake Oswego Police Department at
(503) 635-0238 to report the fraud. Please follow all instructions carefully.
Please note that if there is a confirmed forgery in progress with the suspect still at your
location, immediately call 9-1-1. In this case, no Affidavit is required.
If you have received a check back from the bank that is not payable due to a Closed Account or
Non-sufficient Funds (NSF), and the negotiated amount of the check is under $750, you
may make a report by contacting the Clackamas County Check Enforcement Program at 1-800-671-3049 or
their website at

Checks over $750 will be investigated by the Lake Oswego Police Department. It will be helpful to
the officer investigating the complaint if the above referenced form is completed prior to the
officer's arrival as it will contain the information necessary to investigate the crime, even if it
is not being referred to Clackamas County's Check Enforcement Program.
